Owasippe Adult Forms

We have come to learn Owasippe’s form requirements far exceed our previous camps (which were all in Wisconsin.)

Most of the additional forms are due to the camp’s location in Michigan, which has enacted extensive Youth Protection Standards. Hard to complain about that, but wow. There’s a lot.

At the bottom of the page, you’ll find a pdf with all the forms (some of them fillable) along with a cover sheet to keep track.

Here are the requirements listed out:

For Adults on site 72 hours or more (not necessarily consecutive)

1. BSA Health Form C

For All Adults staying overnight for any number of days:

1. Registered with BSA – Due ASAP
2. IL Child Abuse Registry Check Due 6/1
3. BSA Health Forms A & B with copy of Insurance Card (front & back) Due 6/1
4. Dietary Accommodation Form (if needed) Due 6/15
5. Current BSA Youth Protection Training (YPT) Due 6/15
6. MI YPT and References Form Due 6/15
